Add HR and LR identifiers on Dashboard (Accounts)

ISO would like to see Accounts on the Dashboard to display a number for both high risk and low risk accounts.

As a user, I would like a checkbox on the app portal to select default

This would show as the selected site in the app assistant.

As a merchant, I would like to have the ability to complete my application

Including: - merchant completion of the app - verify the app and T&C - accept the app - fee disclosure - be able to submit app (where it goes depends on what the agent chose on the app assistant)
